Description: The Licensed Practical Nurse (LPN) works as a member of the multidisciplinary health care team, collaborating in the delivery of holistic care and support to patients/residents and families. The LPN practices to full scope, applying professional nursing knowledge, clinical judgment, critical thinking, and communication skills to support safe care and positive health outcomes. The role is committed to providing person-centred, culturally safe, and inclusive care that respects the diverse backgrounds, experiences, values, and needs of patients/residents and families. The LPN contributes to a culture of learning and professional practice excellence by orienting, coaching, mentoring, and supporting the development of staff, students, and learners. Demonstrated leadership abilities. Preferred Qualifications: Recent Continuing Care experience. Connect Care experience. Dementia-specific education (e.g., Supportive Pathways or equivalent) grounded in person-centered care and responsive approaches to dementia-related behaviors. The LPN provides holistic, person-centered care to patients and residents within a multidisciplinary healthcare team. Responsibilities include coordinating care activities, mentoring staff and students, and ensuring patient safety and positive health outcomes.

Candidates must be graduates of a Practical Nursing program with active registration with the CLHA and Basic Life Support certification. Preferred qualifications include experience in continuing care, Connect Care, and dementia-specific education.
