Support the delivery of quality patient care and excellence in professional and clinical nursing practice. Provide leadership by promoting, developing and implementing evidence-based practice. Act as a role model in accordance with the College of Nurses of Ontario Professional Standards and HSN’s Mission, Vision, and Values in both formal and informal learning environments. REPORTING:Under the general direction of the Clinical Manager, Regional Outpatient Geriatric Medicine Service. DUTIES:1. Administer and perform comprehensive geriatric assessments.2. Analyse and synthesize findings from in home geriatric assessment into a comprehensive clinical report identifying client’s priority needs and relevant treatment options designed to optimize their independence. 3. Provide intensive geriatric case management for the medically complex, high risk senior.4. Respond to urgent medical situations in patients’ homes due to unknown nature of patient status.5. Assess condition changes and medication effects and relay relevant information to physicians. 6. Provide support and education to patients and their caregivers.7. Conduct frequent home visits to monitor medical recommendations for unstable geriatric syndromes and chronic conditions made by physicians.8. Review and prioritize all incoming referrals. 9. Manage outreach clinic, support partners in care and provide education and capacity building for health care providers regarding geriatrics.10. Create, implement, and deliver education to medical professionals, nurses, and physicians across the geriatric continuum of care.11. Support the Geriatric Medicine Follow-up Clinic using the Comprehensive Geriatric Assessment; present findings to physicians. 12. Develop, implement and evaluate speciality programs (e.g. bone health and continence).13. Educate client and family in coping strategies, decision-making, and utilization of personal and community resources, and make referrals to community resources as required.14. Collaborate with clients, families, interprofessional team members, and community partners throughout the therapy process and advocate to ensure client needs are met. 15. Complete joint interdisciplinary home visits as required.16. Travel to NESGC sites across Northern Ontario (e.g. Timmins, New Liskeard, Parry Sound, Hearst, Sudbury, Elliot Lake, and James Bay Coast).17. Document clinical impression and all related intervention and patient outcomes in the patients’ medical chart in accordance with HSN policies and the College of Nurses of Ontario. 18. Attend regular in-service sessions, conferences and seminars to keep abreast of new developments, trends and techniques in the nursing field, attending sessions related to specialties as required.19. Participate in studies and projects as required.20. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement. 21. Develop and implement clinical procedures.22. Plan, deliver and evaluate educational programs/content, offering supervision and mentorship through clinical expertise to students through placements and continuing education opportunities.23. Foster a positive learning environment; provide guidance and support the ongoing education of staff. 24. Develop, implement and evaluate staff development/orientation programs based on learning needs, clinical practice standards, treatment trends, technical changes, certification requirements and relevant research findings.25. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.26. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines. 27. Educate and promote health, safety and wellness in the work place.28. Participate on various committees and in meetings as required.29. Perform other duties as required. QUALIFICATIONS EDUCATION AND TRAINING:1. Minimum of a Bachelor of Science in Nursing BSc(N) from an accredited university. 2. Current Certificate of Registration in good standing with the College of Nurses of Ontario.3. Master’s Degree of Science in Nursing or actively pursuing a Master’s is preferred.4. Current Basic Cardiac Life Support (BCLS) certificate is required. 5. Current Automated External Defibrillator (AED) certificate is required.6. Successful completion of Provincial Common Orientation, Core Indigenous Cultural Competency Training Program, and the P.I.E.C.E.S. Training program are required.7. Unit-specific certification, training and education is required. 8. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required. EXPERIENCE:1. Minimum of five (5) years’ experience working within Specialized Geriatric Services and completing assessment and treatment guided by the Comprehensive Geriatric Assessment.2. Minimum of two (2) years recent, relevant teaching experience of clinical adult education programs or equivalent. KNOWLEDGE/SKILLS/ABILITIES:1. Demonstrated knowledge of health assessment, medications, and telemedicine procedures.2. Demonstrated knowledge of health assessment tools (e.g. Comprehensive Geriatric Assessment (CGA), Mini Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Clock Drawing Test, Geriatric Depression Scale, and Cornell Scale for Depression in Dementia). 3. Demonstrated ability to exercise professional judgment, clinical reasoning, and critical thinking with the ability to apply ethical frameworks and choose the optimal assessment and treatment intervention.4. Demonstrated knowledge of mobilizing community health resources and enhancing continuum of care.5. Demonstrated knowledge of education curriculum development, implementation and evaluation. 6. Demonstrated knowledge of Indigenous and Francophone culture.7. Demonstrated knowledge of Specialized Geriatric Centre community services and available resources.8. Demonstrated expertise as an educator using the principles of adult learning. 9. Demonstrated ability to develop, implement and evaluate clinical education methodologies and assessment instruments.10. Demonstrated knowledge and understanding of the relevant acts, regulations, legislation and standards related to Occupational Health and Safety in the hospital setting.11. Demonstrated strong organization skills with the ability to manage a diversified workload, rapidly changing priorities and challenging deadlines. 12. Demonstrated high levels of initiative, self-direction and an innovative approach to problem solving.13. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.14. Demonstrated skills in organizing resources, analyzing and solving problems, establishing priorities and making evaluative judgements. 15. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.16. Demonstrated superior interpersonal and communication skills, both written and verbal.17. Demonstrated discretion and maturity when handling confidential information. 18. Demonstrated commitment to the safety of co-workers and patients. PERSONAL SUITABILITY:1. Successful Vulnerable Sector Check is required.2.
