Position #: 128722 Expected Start Date: TO Be Determined Union: SUN Facility: Meadow Lake Primary Health Care Centre City/Town: Meadow Lake Department: Chronic Disease Prevention Type: Full-time regular FTE: 1 Shift Information: Days Number of Hours per Rotation: 14 shifts of 8 hours per 3 weeks Relief: No Float: No Hours of Work: Field Hours Salary or Pay Band: Pay Band Nurse B $42.040 to $52.440 Travel Required: Yes Job Description: The CDM Clinical Nurse Educator reports to the Manager Clinical Integration and works with an interdisciplinary team with a chronic disease prevention and management focus. The CDM Clinical Nurse Educator combines in-depth clinical expertise, with the principles of teaching and learning, in the provision of self-care management for clients, families, health professionals and students in group and individual settings. Candidates must hold a Bachelor of Science in Nursing (BScN) and be registered with the College of Registered Nurses of Saskatchewan. A minimum of 3 years of clinical experience with chronic disease patients and a Certified Diabetes Educator (CDE) designation are required.
