ABOUT TRAVERSE INDEPENDENCE

Traverse Independence offers an incredible variety of supports and services for those who have an acquired brain injury (ABI), aiding clients to live independently in the community. Traverse Independence was founded in 1975 as The Participation House Project Waterloo Region, an agency dedicated to developing housing with support services for people with physical disabilities. Since then, our organization has evolved from offering services for adults with physical disabilities to serving those with a brain injury in the Region of Waterloo and County of Wellington, across eight facilities. One of these exceptional services is our outreach program. We are dedicated to the mission of maximizing our clients ability to live independently by providing support services for adults with physical disabilities or brain injuries.

A DAY IN THE LIFE OF A COMMUNITY FACILITATOR

As a Community Facilitator you will work in a low barrier program, building trusted relationships with your clients, which in turn will allow you to support them to work on independence in the community. You will provide support to individuals in their homes or in community settings across the Region of Waterloo, and will be responsible for development of client programming, including ongoing assessment, individual program development, implementation and evaluation. As a Community Facilitator, you will demonstrate the ability to work with clients who exhibit various levels of independence and insight into their own abilities. Additionally, you will be required to be a liaison with professionals, community resources, and other significant support persons who are involved with a client.

You will report to a Service Coordinator, in a team that includes regulated staff such as ABI specialized psychiatrists, an Occupational Therapist, Behaviour Therapist and an Intensive Case Coordinator.

COMMUNITY FACILITATOR REQUIREMENTS:

Candidates must have proven organizational skills, interpersonal skills, communication skills (both written an oral), and the ability to work as a member of a multi-disciplinary team with a comprehensive understanding of Microsoft Office products.

Experience working with people with Acquired Brain Injuries is a benefit, but not necessary as we offer a comprehensive training program both internally and through the Ontario Brain Injury Association. This is a great opportunity to add an ABI specialization to your skillset.

Education:

  • Community College Diploma in Social Services, Developmental Services, Health Care, or related discipline.

Knowledge/Skills:

  • Demonstrated experience in behaviour management skills is of benefit.
  • Experience with people with disabilities such as mental health and/or addictions is a great asset but not necessary.
  • Strong utilization and understanding of M365 - Teams/SharePoint.
  • Demonstrated organizational skills, interpersonal skills, and communication skills.
  • Ability to work as a member of a multidisciplinary team.
  • Must have valid driver's license and access to reliable vehicle with proof of $1,000,000 liability insurance and driver's abstract acceptable to Traverse Independence
