The North Lambton Community Health Centre is a community-governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team provides primary health care, health promotion and community development services. The North Lambton Community Health Centre is currently seeking applicants for the following position: 1.0 FTE NURSE PRACTITIONER 37.5 hours/week, full-time with benefits The Nurse Practitioner provides comprehensive, client-centred primary care across the lifespan, including health promotion, disease prevention, diagnosis, treatment, and chronic disease management. Working collaboratively within an interdisciplinary team, the Nurse Practitioner delivers evidence-informed care that supports improved health outcomes and enhances access to high-quality primary care services. Primary location is West Lambton CHC with outreach work integrated into the role. Qualifications: Undergraduate degree in nursing from a recognized university (or equivalent combination of education and experience). Registration in the Extended Class with the College of Nurses of Ontario. Three to five years nursing experience in a community setting or combination of community and hospital or public health settings; experience in a primary care setting, an asset. Proficiency in the use of computers and various software applications. Participates in CNO Quality Assurance Program. Has and maintains CNO Professional Liability Insurance. This is a full-time position, working 37.5 hours per week, Monday to Friday at our West Lambton location in Sarnia. Experience will be taken into consideration on our exciting new 6-step wage grid.
