Subsidized RNAO & RPNAO memberships Mobile device and 3 uniforms provided Responsibilities Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions. Support the client through care transitions by collaborating and connecting the client with community support services in their local community. Requirements Current Certificate of Competence with the College of Nurses of Ontario. Availability to work PT Monday-Friday, 2-6pm at 122 King St in Hensall, ON. Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire. A willingness to attain required SE Health certifications A current CPR certificate is required. This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
