Geriatric Ambulatory Access Office Team Parkwood Institute Main - London, ON Full Time Salary Range: $41.15 - $58.98 /hour St. Joseph’s Health Care is willing to consider applicants who are eligible for Community Commitment Program for Nurses (CCPN). This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two-year full-time commitment. The Registered Nurse with the Geriatric Ambulatory Access Team will be part of an exciting team that supports patients/family caregivers in accessing Community Geriatric Services through assessment and triage by telephone assessment in an office environment. Essential Qualifications Valid registration with the College of Nurses of Ontario Valid CPR certificate Previous experience with assessments in an inpatient or community-based geriatric population Excellent problem-solving skills and ability to prioritize in situations of varying degrees of complexity Ability to function as an independent practitioner and a highly effective team member Knowledge of how to mobilize community health resources and systems Ability to synthesis clinical information succinctly and accurately both written and verbally Exceptional verbal and written communication skills with a variety of community and organizational partners Demonstrated application of case management skills in complex clinical situations Superior interpersonal and relationship building skills Ability to be flexible in the scheduling of work hours Exceptional computer skills Exceptional conflict management skills Demonstrated ability to use best practice and research Strong ability to think critically around clinical issues Strong ability to communicate/transfer clinical summaries successfully Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA Preferred Qualifications BScN Experience with Cerner and Clinical Connect Knowledge of registration and scheduling Teaching and Research St. Joseph's Health Care London through its affiliation with Western University and Fanshawe College is a leading research and teaching hospital.
