Visit us at https://welldiagnostics.ca Responsibilities: * Conduct comprehensive health assessments, including medical histories and physical examinations in collaboration with cardiologists and Internal Medicine specialists. * Provide patient education and counseling regarding disease prevention, treatment options, and healthy lifestyle choices. * Monitor patient progress and adjust treatment plans as required. * Collaborate with physicians, specialists, allied health professionals, and community resources to ensure continuity of care. * Maintain accurate and timely clinical documentation within the electronic medical record (EMR) system. * Participate in quality improvement initiatives and support best practices in patient care. * Adhere to all regulatory, professional, and organizational standard * Perform consultations with patients as assigned. * Perform stress test supervision as assigned. * Perform all medical tasks delegated by the attending physician. * Contribute to ensuring there is a safe environment for patients, staff and visitors. * Maintain safe and clean working environment by complying with procedures, rules, and regulations. * Call for assistance from health care support personnel, if required. * Document patient care services by charting in patient records. * Take all patient vitals and history. * Maintain patient information electronically. * Document and record medicine and prescriptions. * Perform any and all other tasks as assigned by the supervisor. Qualifications, Experience & Requirements: * 0-2 years of related experience. * Nurse Practitioner certification is required. * Member in good standing with the College of Nurses of Ontario. * Demonstrated knowledge and understanding of general medicine. * Demonstrated ability to collaborate and communicate effectively with health professionals and patients. * Must possess the ability to function effectively in a clinical setting. * Familiarity with electronic medical records (EMR). * Knowledge of and demonstrated ability in corporate core competencies including customer service, communication, teamwork, initiative/self-management, accountability, dependability, flexibility and adaptability. * Computer literacy utilizing MS Office software applications and ability to research on the Internet. * Ability to work within a diverse community and to deal empathetically with patients, family members, etc., to contribute to their well-being. * Ability to work effectively independently or as a part of a multidisciplinary healthcare team. * Demonstrated commitment to continuing education and training. * Demonstrate effective decision-making and problem solving skills.

Exigences
